Why does my laptop screen go fuzzy?

When my monitor turns off after periods of activity and goes back to active, the monitor is unreadable. It looks like one huge blur and nothing is readable. I have to reboot to fix the problem, and that doesn't always work. Also, when I adjust the angle of my screen lines show up on the monitor while I'm moving it. What's going on and how do I fix it?
